Each state varies in what is allowed as far as what companies
will reimburse you for massage therapy. While I can't tell you what to do
in your state, you can find out by learning about the rules and asking each
company about their reimbursements for massage therapy.

Companies - I am collecting information on what billing is allowed in

The ability to bill insurance
companies? How can you get recognized in your state? Do you really want to
be recognized?
HCFA - Word Doc. Use a Word template to complete a
HCFA form. Just type in the gray areas and fill out in the same manner as
a HCFA and save to your computer. Print on a HCFA. (
